COVID-19 Alert Level 4 Form

As we move through alert level 4, the Government are adjusting what is and isn’t deemed an essential service.

NZ Post is an essential service and under alert level 4 can only provide services to other essential businesses. To see if you are an essential service, please carefully consult the latest Government list of essential businesses, products and services here.
 
In order to comply with Government requirements, we need you to clarify if your business is an essential service and what essential services you are providing. Please carefully read and complete the declaration form below.
I understand that only businesses classified as essential by the New Zealand Government may send items through NZ Post’s network. I also understand that only essential items may be sent through NZ Post’s network. I am aware that sending non-essential items will reduce NZ Post’s ability to deliver items necessary for the provision of life. It is critical that New Zealand Post is able to support: a) the essential services required for society to operate during this period; b) the protection of its staff and contractors from unnecessary risk of exposure to COVID-19; and c) the ongoing measures in place to stop the transmission of COVID-19. Any non-essential delivery compromises that support. With that knowledge, and in making the declarations contained in this form, I explicitly confirm that I have been accurate and truthful in all respects. *

If NZ Post has reason to believe that the information submitted in this form is incorrect, we will immediately suspend your use of our services for the length of time deemed necessary. We may exercise our right to terminate services, if required.
